In a new real-world analysis, Counterpart Health compared members attributed to Counterpart Assistant (CA)-enabled PCPs with
@CloverHealth members attributed to PCPs not using CA, examining performance on high-risk care transition outcomes, including two HEDIS® quality measures.
Among the findings, CA-enabled PCPs showed better HEDIS® post-acute performance, including 33% higher discharge information receipt and an 11% higher rate of post-discharge medication reconciliation, underscoring the role of AI-enabled workflows in high-risk care transitions.
