Furthermore, A 2024 critical review notes how LLMs miss nonverbal cues and may underestimate suicide risk, meaning that seemingly empathic text can fail where skilled therapists would intervene (Kalam et al., 2024)

The key lies in modularity. The diagram below shows two very different feedback loops: standard LLMs rely on users to manually disclose emotional context, offering one-off advice that resets every conversation, while an emotionally intelligent system passively senses signals, infers state, adapts its response, and remembers—shifting the emotional labor from user to machine.

According to a recent survey conducted by Sentio University, over 50% of people with mental health challenges have turned to LLMs like ChatGPT for emotional support or informal therapy.

The surveys sample included 499 U.S. adults with ongoing mental health conditions, and was conducted via Prolific (Feb 2025). Among the participants: -54.5% said the response they received was factually incorrect -45.5% said it was dismissive or minimizing -41% said it was offensive or insensitive This kind of mismatch reveals just how much nuance LLMs overlook users’ emotional context.

Today’s LLMs require manual disclosure: For example, you’d have to manually prompt, “I’m avoidant, and I’m not sure how to communicate my need for space to my friend." Requiring users to clarify their emotional state is reactive and impractical. A truly intelligent system should infer emotional cues from language and adapt its guidance accordingly.

Let’s put it to test with ChatGPT: Prompt: “My partner hasn’t texted me back all day. What do I do?” Looks supportive right? But wait, here’s the gap: LLMs give the same advice whether you’re anxiously spiraling or emotionally withdrawing. However, in attachment theory, those are opposite nervous system states that call for completely different responses.

This is important because your attachment style decides the degree of responsiveness required from the LLM, e.g. individuals with higher anxious attachment were more likely to seek emotional support online during distress, including through apps and digital tools (Heshmati et al., 2020).

So while anxious users might over-rely on AI, avoidant users might disengage completely, raising opposite design challenges for emotionally intelligent systems.

Additionally, A 2025 peer-reviewed perspective summarizing an MIT OpenAI RCT reports that while loneliness dropped on average, extended daily chatbot use was associated with decreased socialization, which shows a potential dependency risk (Ophir, 2025). PMC Over-positivity from AI can foster dependency, as humans naturally seek praise, leading them to overuse a system that offers empty reinforcement.

So why do most LLMs have a tendency to over-validate their users? This is because they are trained with human feedback that often shows sycophancy, a concept of agreeing with users and mirroring their beliefs to please them, even at the expense of accuracy (Sharma et al., 2023).

But, a landmark meta-analysis found over one-third of feedback interventions actually decreased performance, showing that poorly targeted or inflated feedback can harm learning (Kluger & DeNisi, 1996). Learning is an essential necessity in therapy, as it allows individuals to grow, adapt, and develop the self-awareness and coping strategies needed for lasting healing and personal transformation.

Sycophancy poses a concrete risk in advice giving contexts. A Stanford analysis found that therapy chatbots sometimes enabled dangerous prompts like responding to suicidal ideation with practical details, instead of challenging the thinking, highlighting safety-critical failures of “agreeable” bots (Stanford HAI, 2025)

When praise exists only to inflate one’s ego? (“You’re so great!”) Rather than challenge their preconceived notions, people become less antifragile (willingness to persevere during adverse circumstances) to protect the self-serving compliment at all costs (Mueller & Dweck, 1998).

So what’s the difference between a standard LLM advice-giving approach vs. a truly emotionally intelligent system?

Conversely, avoidant attachment consistently predicts disengagement from digital help. Gillath et al. (2021) found that avoidantly attached individuals showed significantly less trust in AI agents, especially compared to secure or anxious individuals. Moreover, Adams et al. (2018) showed that people with high avoidant attachment were significantly less likely to seek help at all (P < .05), including from digital sources.
